How to administer MDI-Metered dose inhaler

The procedure wherein an MDI device used which delivers a measured amount of medication as a mist that the patient can inhale. An MDI consists of a pressurized canister of medication in a case with a mouthpiece.

Material needed

Washbasin or sink to rinse the mouth

PROCEDURE

Confirm the patient carefully as follows

Ask the patient his/her name or call out the patient’s name and re-confirm

Check the name & the Unique hospital ID on the identification band

Confirmation of the medication (drug notes)

Check the name, dosage, form of medicine, time of administration with the patient’s drug card.

Check the drug card and the patient’s record for any allergies to medications

Perform hand hygiene using Hand based sanitizer

Arrange all the required articles

Explain the procedure to the patient

Procedure

Allow the patient to hold and manipulate the canister and explain how the canister fits into the inhaler.

Have the patient demonstrate the insertion of the canister

Discuss the metered-dose concept and frequency of dose to the patient

Explain that the inhaler should be shaken before each use

Remove the mouthpiece and cap from the bottle and insert the stem into the small hole on the flattened portion of the mouthpiece. The patient should grasp the inhaler with thumb and first two fingers

Instruct the patient to exhale, place the mouthpiece into the mouth, and tighten the lips (seal) around the mouthpieces

Ask the patient to firmly push the cylinder down against the mouthpiece only once, while slowly inhaling until the lungs feel full

Instruct the patient to remove the mouthpiece while holding the breath for about 10 seconds then exhale slowly through pursed lips

Repeat the doses as prescribed, with a one minute pause between puffs

A mouthwash can be used by the patient to remove the taste of the medication

Demonstrate to the patient how to wash the mouthpiece under running water to remove secretions

If two or more inhaler medications are prescribed, wait 5-10 minutes between inhalations.

Record all the drug’s name, dose, date, and time, with the signature of the nurse for the medication in the drug card matches with the clinical record as appropriate

Observe for the effectiveness of medication and relief of the patient’s symptoms & document the details.

How to administer MDI in ventilator patients

Disconnect the ventilator connection from the Endotracheal tube. connect the metered dose inhaler to ET tube and puff the Salbutamol dose required and reconnect to ventilator.
